What are hemorrhoids?

A precise definition of hemorrhoids does not exist, but they can be described as masses or clumps ("cushions") of tissue within the anal canal that contain blood vessels and the surrounding, supporting tissue made up of muscle and elastic fibers. The anal canal is the last four centimeters through which stool passes as it goes from the rectum to the outside world. The anus is the opening of the anal canal to the outside world.

Although most people think hemorrhoids are abnormal, they are present in everyone. It is only when the hemorrhoidal cushions enlarge that hemorrhoids can cause problems and be considered abnormal or a disease.

Prevalence of hemorrhoids

Although hemorrhoids occur in everyone, they become large and cause problems in only 4% of the general population. Hemorrhoids that cause problems are found equally in men and women, and their prevalence peaks between 45 and 65 years of age.

Anatomy of hemorrhoids

The arteries supplying blood to the anal canal descend into the canal from the rectum above and form a rich network of arteries that communicate with each other around the anal canal. Latest Reichs novel has Montreal appeal

Flash and Bones by Kathy Reichs (Scribner, 278 pages, $29.99)

NASCAR and Bones: the timing could not be more perfect.

Montreal has just played host to the stock-car race and one of its own - parttime resident Kathy Reichs - weighs in with another of her Temperance Brennan novels.

Reichs, who has parlayed the success of her books into a very lucrative television show called Bones, puts the emphasis this time on race car culture.

The 14th in the series, this novel is set in Charlotte, North Carolina. As always, the narrator is Tempe, a forensic anthropologist who divides her time between the medical examiner's offices in Charlotte and the Quebec provincial police division in Montreal.

Just prior to race week, a barrel is found in a landfill adjacent to the track. It's full of asphalt and contains a body. Tempe is asked to figure out who was buried and when.

A pit crew member by the name of Wayne Gamble comes to see Tempe when news comes out about the discovery. His sister, Cindi, and her boyfriend, Cale Lovette, both disappeared 12 years ago. Could the remains in the barrel be either Cindi or Cale?

Seems Cale was part of a political extremist group back in 1998. The police report at the time suggested the duo skipped town to get married. Wayne thinks not.

Tempe examines the remains and then the body disappears, as do all the records, both current and those a decade old. Hmmm, why is the FBI removing all traces of this crime?

Along for the ride with Tempe are a large detective known as "Skinny" Slidell and a charismatic disgraced cop, Cotton Galimore.

Reichs seems to be having a bit of fun 14 books in: Tempe observes that she'd rather have hemorrhoids than speak to her ex's new fiancee. There are scary bits and, as with every one of her books, Tempe is in danger at some point.And the parts about Jacques Villeneuve will appeal especially to Montrealers.

Ilene Beckerman of Bethlehem Township writes memoir of her Jewish grandmother

Ilene Beckerman of Bethlehem Township has released a new book, “The Smartest Woman I Know.” It is about the grandmother with whom she spent her teen years.

How smart was her grandmother?

-- When young Ilene discovered she needed eyeglasses and braces: “It’s not the end of the world. There’s already been a Jewish Miss America (Bess Meyerson, 1945). Now we need a Jewish woman President — so go do your homework!”

Although the book spares the reader the grim details, when Beckerman was 12, her hard-working mother died of a stroke and “my grandparents had no love for my father and they just took us.” Until she went off to college, Beckerman lived with her grandparents, proprietors of a Manhattan candy store on Madison Avenue between 64th and 65th streets.

Her grandmother enjoyed working in the store and bantering with the customers, one of whom was Sara Delano Roosevelt, who lived nearby. “Sara worried about her son Franklin’s future because of his polio… ‘Don’t worry,’ Ettie told Mrs. Roosevelt, ‘your son’s got a good head on his shoulders. I bet someday he’ll be president.’ Ettie told that to every customer who had a son. ‘That’s how you make a customer,’ she told me.”

The little, 100-page volume, published by Algonquin Books, is cleverly illustrated with a mix of photos and drawings.

Beckerman will be signing copies of it at the Clinton Bookshop on Tuesday, Sept. 6, at 7 p.m. and at The Bookworm on Saturday, Sept. 24, 2-3:30 p.m. in Bernardsville.

Beckerman, who is 75, lives near Hampton with her husband, Stanley, “in an old barn that someone put together with found material. I love it; there’s not a right angle in the whole place.” They have lived there for 14 or 15 years.

She scored a bulls-eye in 1995 with her first book, “Love, Loss and What I Wore.” The New York Times said, “This small gem is worthy of a Tiffany box.” It has since been make into a play that is currently being performed in New York City and Chicago.

Beckerman recalls, “I wrote my first book for my children; they didn’t think I had a life before them.” She made seven copies — one for each child and one for herself, “and somebody sent the book to the publisher. Then somebody called me.

“When my first book came out, my children were so proud of me.” Beckerman felt that the kids needed to see their mother get “some validation.” When she was raising her family in Livingston “mine was the house where, when the children and their friends wanted to fingerpaint, other parents would say, ‘Go to Mrs. Beckerman’s; she doesn’t care.”

After her first book, she went on a book tour. “I think because I was older, women could talk to me; they didn’t feel threatened. I met wonderful women across the country and they were asking me for certain kinds of answers. They asked for my second book. So she examined “all the myths I grew up with. I grew up when the music was Sinatra and you heard the words and you believed them,” and in the movies you saw Fred Astaire and Ginger Rogers falling in love, and the myth of true love was perpetuated. So she addressed that in “What We Do for Love.” Her grown children’s excitement over her continued literary career dwindled when Mom wrote about her love life.

R. Crumb and the Power of Bad Press

Courbet embraced this uncivilized persona, played along with his critics, and even cultivated outrage. Anything that causes a stir, after all, brings publicity. In 1855, when the Salon—the official art exhibition in France—accepted several of his paintings except one, "The Artist's Studio," Courbet put up an oppositional show called "The Pavilion of Realism." He repeated the gesture in 1867, when he showed about 140 works in a personal exhibition on the Place de l'Alma. In doing so, Mansfield says, "He created a rejection of his work that didn't really exist."

But Courbet also felt the sting of criticism. When the Salon refused to show his painting, "Venus and Psyche," on account of its "indecency," Courbet was left in the dark, frustrated and anxious. In 1864 he wrote in a letter, "It is impossible for me to continue to exhibit. It is a tricky business. These people want revenge at all cost... With all these troubles, I have internal hemorrhoids that are killing me. I am literally unable to work. I am into leeches, into baths, armed with a syringe, and I am up all night."

Edouard Manet felt similarly despondent after his painting, "Olympia," was shown at the French Salon in 1865. Reviewers said it "recalls the horror of the morgue," and that "the color of the flesh is dirty, the modeling non-existent." Others wrote, "I do not know whether the dictionary of French aesthetics holds expressions to characterize her...her face [is] stupid, her skin cadaverous," and the artist is "a brute who paints green women with dish brushes." Young or pregnant women were warned that they should "flee this spectacle."

But a couple years later, Manet put on his own anti-Salon show. The catalogue stated: "The first stage in an artist's career is a battle, which at least should be fought on equal terms, that is to say that the artist should be able to show the public what he has done." Otherwise, "he would be forced to make a pile of his canvases or roll them up in the attic... Monsieur Manet has never wished to protest. On the contrary, the protest, entirely unexpected on his part, has been directed against himself...By exhibiting, an artist finds friends and allies."

When defying critics in the showroom won't suffice, other artists have gone to court. American artist James Whistler sued John Ruskin, who reviewed his 1875 work "Nocturne in Black and Gold: The Falling Rocket," by condemning the painting, the artist, and the gallery. "The Grosvenor Gallery ought not to have admitted works into the gallery in which the ill-educated conceit of the artist so nearly approached the aspect of willful imposture," Ruskin wrote in the November 1878 Fors Clavigera newsletter. Whistler won his case, although wound up losing money on legal fees after he was awarded only a farthing.

Real-life tragedy shouldn't be reality-TV entertainment

"Don't miss a moment of the high fashion, high drama and hijinks as 'Real Housewives of Beverly Hills' returns for Season 2. Join the pampered princesses of the world's most famous ZIP code as they struggle with questions all the money in the world cannot answer. Can sisters Kim and Kyle repair their broken relationship after last season's fight? Can Camille find happiness in her new life without Kelsey? And, what will Taylor do when she finds out her estranged husband committed suicide after seeing his private life played out as a cheesy soap opera to sell hemorrhoids medicine and feminine hygiene products to a mass audience?"

Maybe you find the foregoing an unsuitably cynical response to last week's news that a fellow named Russell Armstrong hanged himself and that people around him blame it on the pressures of seeing his wife, reality star Taylor Russell, file for divorce as his finances crumbled (the Los Angeles Times calls him a "struggling entrepreneur" with a $12 million debt) and an audience looked on.

But it seems to me the real cynicism is embodied in a TV show's decision to treat people's actual lives and misfortunes as entertainment.

"'The Real Housewives of Beverly Hills,' I think, was (Russell's) downfall," a friend named William Ratner told the Los Angeles Times. "The TV show put a lot of pressure on him to produce financially. You're on a show with a couple like the Maloofs, who are verifiable billionaires, and you're not."

The reference was to "Housewife" Adrienne Maloof, whose family owns the Sacramento Kings basketball team.

"The program itself," Russell's mother, John Ann Hotchkiss, told CNN's Headline News, "just really brought him down." She said her son was constantly bashed on the show and could do nothing about it because the conflict generated ratings. "All the network cares about are ratings. They don't care how people feel." Nor is Russell reality TV's first meltdown. Jon and Kate Gosselin got divorced. Singer Fantasia Barrino tried to commit suicide. Danielle Staub thought about it.

"I was very close to taking my own life — not just on one occasion — it's been several times," the former cast member of "Real Housewives of New Jersey" recently told "Entertainment Tonight." "I don't have words to describe how alone you feel when everybody's coming at you and judging you — and they don't even know you."

Now there's this. And you wonder if, even at this price, we will finally realize that other people's lives are not a car crash we slow down to watch on the freeway.

This brand of television is the moral equivalent of those "bum fight" videos where homeless men are paid to scrap on camera and the video is posted online. There is something similarly predatory in searching out these troubled people, these drunks and narcissists, these self-centered, superficial plastic surgery junkies, these screechers and whiners and perpetual adolescents with daddy, esteem or anger management issues, and paying them to let us watch as they implode. There is something reprehensible in the watching, too.

Leonard Pitts Jr., in his Miami Herald column, takes a look at problems surrounding reality TV in light of the suicide death of the estranged husband of a star of Real Housewives of Beverly Hills.

One imagines the promo will pretty much write itself.

Anatomy of hemorrhoids

The arteries supplying blood to the anal canal descend into the canal from the rectum above and form a rich network of arteries that communicate with each other around the anal canal. Because of this rich network of arteries, hemorrhoidal blood vessels have a ready supply of arterial blood. This explains why bleeding from hemorrhoids is bright red (arterial blood) rather than dark red (venous blood), and why bleeding from hemorrhoids occasionally can be severe. The blood vessels that supply the hemorrhoidal vessels pass through the supporting tissue of the hemorrhoidal cushions.

The anal veins drain blood away from the anal canal and the hemorrhoids. These veins drain in two directions. The first direction is upwards into the rectum, and the second is downwards beneath the skin surrounding the anus. The dentate line is a line within the anal canal that denotes the transition from anal skin (anoderm) to the lining of the rectum.

Formation of hemorrhoids

If the hemorrhoid originates at the top (rectal side) of the anal canal, it is referred to as an internal hemorrhoid. If it originates at the lower end of the anal canal near the anus, it is referred to as an external hemorrhoid. Technically, the differentiation between internal and external hemorrhoids is made on the basis of whether the hemorrhoid originates above or below the dentate line (internal and external, respectively).

As discussed previously, hemorrhoidal cushions in the upper anal canal are made up of blood vessels and their supporting tissues. There usually are three major hemorrhoidal cushions oriented right posterior, right anterior, and left lateral. During the formation of enlarged internal hemorrhoids, the vessels of the anal cushions swell and the supporting tissues increase in size. The bulging mass of tissue and blood vessels protrudes into the anal canal where it can cause problems. Unlike with internal hemorrhoids, it is not clear how external hemorrhoids form.

Bioengineered anal sphincter could transform fecal incontinence care

What's best for a wrecked rectum? Maybe an auxiliary anus.

Kidding aside, scientists say they've created the world's first replacement anal sphincter, the circular muscle that rings the rectum - and predict it could make an effective treatment for fecal incontinence. That's potentially big news for people suffering from the embarrassing condition, given the limited effectiveness of the electrical implants, muscle grafts, and other treatments now in use.

"In essence, we have built a replacement sphincter that we hope can one day benefit human patients," study author Dr. Khalil N. Bitar, a professor at Wake Forest Baptist Medical Center's Institute for Regenerative Medicine, said in a written statement.

Using human muscle and nerve cells, scientists were able to make the bioengineered sphincters in about six weeks. The sphincters worked normally when implanted in mice. The ultimate goal of Bitar's effort is for doctors to take muscle and nerve cells from individual fecal incontinence patients and use them to create "personalized, hard-wired" sphincters that would not be rejected by an immune response.

The research was detailed in the July 2011 issue of the journal Gastroenterology.

Calling his work "proof of concept research," Dr. Bitar said bioengineered sphincters might prove useful in the treatment of urinary as well as fecal incontinence, a term that covers both the inability to hold a bowel movement until reaching a bathroom and the accidental leakage of solid or liquid stool while passing gas.

Nearly 18 million U.S. adults have fecal incontienence. Risk factors for the condition include nerve or muscle damage, hemorrhoids, poor overall health, and injury to the "pelvic floor" tissues during childbirth.

China Sky One Medical Announces Second Quarter 2011 Results

"Our second quarter revenue declined 7.6% year-over-year, primarily reflecting the loss of two distribution relationships in the third quarter of 2010. We continue to aggressively pursue new customers to distribute our broad portfolio of pharmaceutical products, while investing in China Sky One's future, as exemplified by our winning bid on land use rights for land in Harbin's Song Bei District. We intend to build a research and development center, an injection manufacturing facility, a logistics center and an office building on the land during the first phase of development, which we expect to complete by mid-2012," said Mr. Yan-Qing Liu, Chairman and CEO of China One Medical, Inc. In the second quarter of 2011, China Sky One's total revenues decreased 7.6% to $37.7 million from $40.8 million in the same quarter last year, largely reflecting the continuing impact of terminated business relationships with a domestic distributor and an overseas sales agent during the third quarter of 2010, which negatively impacted the sales of the Company's Ointments, Patches, Sprays and Diagnostic Kits categories. These two distributors accounted for 0% and 17.4% of the Company's overall revenue in the second quarter of 2011 and second quarter of 2010, respectively.

By product category, lower sales from Ointments, Patches, Sprays and Drops contributed to the Company's overall year-over-year revenue decline, offset somewhat by top-line growth in Wash Fluids, Dignostic Kits, Suppositories and the Others category.

Sales of ointments declined 28.5% year-over-year to $8.3 million in the second quarter of 2011, primarily due to a $2.6 million year-over-year decline in the Company's Hemorrhoids Ointment product.

Sales of patches declined 34.2% year-over-year to $6.4 million in the second quarter of 2011. The decrease in patch product sales was primarily due to a 63.3% decrease in Slim Patch sales, along with sales declines of several other Patch products, primarily due the termination of a key distributor relationship in the third quarter of 2010.

Sales of wash fluids increased 133.5% year-over-year to $4.3 million in the second quarter of 2011, driven by strong sales of Metronidazole and Chlorhexidine wash fluids.

Had BACH health care providers diagnosed

U.S. District Court Judge John Nixon in Nashville on Friday approved the settlement of the medical malpractice claim made by Staff Sgt. Adam Cloer, of Missouri, on behalf of his wife Melodee Cloer, who died last year after being diagnosed with rectal cancer. The settlement is subject to final approval by the U.S. Attorney General.

The lawsuit claimed that medical staff at Blanchfield Army Community Hospital at the installation on the Tennessee-Kentucky state line failed to screen her for rectal cancer despite persistent symptoms. The lawsuit said her cancer spread and despite multiple surgeries, she died in May 2010 at the age of 53.

The U.S. Attorney representing the government, Sam Delk Kennedy Jr., declined to comment when reached by The Associated Press on Monday. The settlement is not an admission of liability or fault.

The lawsuit, which was initially filed in 2009, said Melodee Cloer first went to the hospital at Fort Campbell with symptoms in 2006 complaining of pain, blood in her stool and constipation. The lawsuit said she went to the hospital on multiple occasions complaining of symptoms, but was diagnosed with hemorrhoids. A bariatric surgeon recommended that the hospital perform a colonoscopy, but the lawsuit claims the hospital failed to follow-up on that recommendation or provide any rectal cancer screenings.

In 2007, her husband got orders to move to a new Army installation, Fort Leonard Wood in Missouri, where she learned that the cancer had spread to her lymph nodes. She underwent chemotherapy and radiation therapy and several surgeries to remove some of her organs.

After her death in 2010, Staff Sgt. Cloer filed an amended complaint arguing that the hospital's negligence in diagnosing and treating her cancer resulted in her death.

"Had BACH health care providers diagnosed and treated Ms. Cloer's cancer at any time from the start of her rectal cancer symptoms in early 2006 through the end of 2006, then more likely than not, Ms. Cloer's cancer would have been curable; her multiple and painful surgeries requiring removal of some of her organs would have been avoided; and she would be alive today," the complaint said.

The couple has two children and Melodee Cloer has a daughter from a previous marriage.

The federal government denied the malpractice claims in court records and argued that the lawsuit was filed too late under Tennessee's medical malpractice statutes. But the two sides agreed to a settlement before a scheduled July bench trial was set to begin.

Cloer's attorney, Jamal Alsaffar, declined to comment on the settlement when reached Monday by The Associated Press.

Doctor pleads guilty to 40 felony counts of fraud

A doctor has pleaded guilty to 40 felonies in a notorious medical insurance fraud case that involved healthy people being recruited to undergo unnecessary surgeries in exchange for money or low-cost cosmetic procedures such as "tummy tucks."

Michael C. Chan, 65, of Cerritos, former owner and medical director of the Unity Outpatient Surgery Center, will spend six months in jail and faces additional time behind bars at his upcoming sentencing.

He is among 19 defendants charged with orchestrating what prosecutors say is the largest medical insurance fraud scheme in the nation, a $154-million scam that was centered out of the now-shuttered clinic in Buena Park,

As part of a plea agreement, Orange County Superior Court Judge Thomas Goethals on Thursday ordered Chan to pay up front $250,000 for his role in the scheme, and said he would have to make full restitution eventually.

Chan faces anywhere from probation to 28 years in state prison after he serves six months in jail. He is out of custody until his sentencing Sept. 9.
